ABSTRACT

Objective To explore the rule of transportation in extracellular space (ECS) and the changes induced by the external stimulation with tracer-based MRI.Methods Thirty two mature Sprague Dawley rats were randomly divided into four groups,i.e.caudate nucleus-control (Cc) group,thalamus-control (Tc) group, caudate nucleus-moving (Cm) group and thalamus pain stimulation (Tp) group. The rats were anesthetized and a series of MR scanning were performed before and after the injection of Gd-DTPA in ECS of caudate nucleus and thalamus until the intensity of Gd-DTPA was invisible.Half-life of Gd-DTPA in ECS was calculated and analyzed with t-test.Results Gd DTPA in caudate nucleus was transported to the ipsilateral cortices away from the injection points, which only distributed on site in the thalamus. The transportation between the two partitions was not observed. The half-life of Tc group ([49.93±2.11] min) was significantly shorter than that of Cc group ([104.30±54.12];t=2.839,P<0.05),no difference was observed between the Cm group ([113.42±47.32]min) and Cc group (t=0.359,P>0.05),while half-life of Tp group ([109.40±10.33]min) was significantly longer than that of Tc group (t=15.954,P<0.05).Conclusion Tracer-based MRI can be used to investigate the rule of transportation in ECS,and the transportation and clearance of substances into the ECS can be influenced by a selective external stimulation.

ABSTRACT

Objective To study the gene mutations and clinical features of mandibuloacral dysplasia with type A lipodystrophy (MADA) in a Chinese family. Methods The information of 5 family members including 2 siblings suspected atyp-ical progeria was assembled. Genomic DNA was extracted from peripheral blood of 5 family members, the 12 exons of LMNA gene were ampliifed by PCR and then the PCR products were directly sequenced and analyzed by using Blast software online. The SIFT and PolyPhen-2 software were used to predict the harmfulness of mutations. Results The 2 siblings were clinically diagnosed as MADA. Heterozygous c.1579C>T (p.Arg527Cys) and c.1583C>T (p.Thr528Met) mutations were detected in this family. The father carried c.1583C>T (p.Thr528Met) mutation, the mother carried c.1579C>T (p.Arg527Cys) mutation, and their normal daughter were all heterozygous carriers with c.1583C>T (p.Thr528Met) mutation. Compound heterozygous c.1579C>T (p.Arg527Cys) and c.1583C>T (p.Thr528Met) mutations in 2 siblings led to MADA. The MADA showed an autosomal re-cessive inheritance pattern in this family. Conclusions The 2 siblings with MADA in this family were caused by compound heterozygous mutations in LMNA gene.

ABSTRACT

Objective To study the variation characteristics of peripheral blood β-catenin and DKK1 levels in patients with rheumatoid arthritis (RA) and the correlation with bone and joint damage. Methods One hundred and eight patients with RA (observation group) and 110 healthy individuals (control group) were selected. The bone mineral density(B MD) in the femur (femur neck,Ward area, greater trochanter, total femur area) and lumbar spine (L2,3,4) were measured by dual-energy X-ray absorptiometry instrument.X-ray for two hands were evaluated according to the Sharp score. The peripheral blood β-catenin and DKK1 levels were detected by enzyme-linked immunosorbent assay (ELISA).Multiple analysis was completed by multiple linear regression and Logistic regression. Results The peripheral blood DKK1 level in observation group was significantly higher than that in control group [(8.2 ±5.0) mg/L vs.(6.1 ±4.2)mg/L],there was statistical difference (t =2.452,P =0.025),but there was no statistical difference in β-catenin level between the 2 groups (t =15.947,P =0.142).Compared to control group, patients with RA had lower BMD at femur and lumbar spine (P < 0.01).The incidence of osteoporosis in observation group was significantly higher than that in control group [31.5% (34/108) vs.15.5% (17/110)],there was statistical difference (x2 =9.289,P =0.005).The correlation analysis results showed that the peripheral blood DKK1 was positive correlation with erythrocyte sedimentation rate (ESR),28-jonit disease activity score (DAS28),alkaline phosphatase (AKP) and two hands X-ray joint narrowing space score in patient with RA (P < 0.01 or < 0.05) ;the peripheral blood β-catenin level was positively correlated with ESR and AKP (P < 0.01 or < 0.05).The multiple linear regression results showed that the disease duration, health assessment questionnaires (HAQ),DKK1 influenced two hands X-ray joint narrowing space score; but the age, disease duration,HAQ influenced two hands X-ray bony erosion score. The multiple Logistic regression results showed that the two hands X-ray Sharp score was the risk factor of osteoporosis at femur in patients with RA; but the age was the risk factor of osteoporosis at lumbar in patients with RA.Conclusions Peripheral blood DKK1 level in patients with RA increases significantly, while there is no apparent alteration in β-catenin level.Peripheral blood DKK1 level is correlated with disease activity and joint space narrow score.

ABSTRACT

Objective:To investigate the clinical characteristics of renal injury caused by Guanxinsuhe pill(冠心苏合丸,including slander dutchanspipe root(青木香)).Methods:The clinical data of 27 patients with renal injury caused by Guanxinsuhe pill were studied. The clinical characteristics of the renal damage caused by Guanxinsuhe pill and the relationships between duration of illness, duration of taking Guanxinsuhe pill, anemia, renal atrophy and the progression of renal injury respectively were also analyzed.Results:Twenty-seven patients all taken Guanxinsuhe pill for suspicious coronary heart disease(CHD) and were diagnosed as chronic tubulointerstitial nephropathy accompanied with renal tubular acidosis, renal glucosuria and hyposthenuria after admission.Anemia was of more severity than renal dysfunction.Atrophic kidney occurred in 26 patients. The main complains were nonspecific symptoms including fatigue, anorexia, nausea, vomiting, etc.and accompanied with different degrees of chronic renal dysfunction.Twentyfour hours urinary protein excretions were 120-900 mg((490?250) mg). There were few red and white cells in uroscopy.Sixteen patients discontinuously took the Guanxinsuhe pill(one capsule,2-3 times a day) according to the pharmacopoeia for a long period.Ten patients took a dosage(two capsules,2-3 times a day) higher than that limited by the pharmacopoeia.Twenty-six patients took Guanxinsuhe pill discontinuously. One patient continuously took Guanxinsuhe pill until got renal dysfunction. The duration of taking Guanxinsuhe pill was 1-120 months(37.38?31.58),and the amount of 50-43 800 pills(4 576.00?8 830.54) were used, but the amount of pills of 3 cases was uncertain. The renal injury occurred within 30-216 months(95.41?56.74) after initiation of the drug taken. The renal injury obviously correlated with the anemia and the duration of taking Guanxinsuhe pill. Conclusion:Taking Guanxinsuhe pill can cause renal injury. The Guanxinsuhe pill dose indicated in pharmacopoeia also causes renal damage. The onset of renal injury is insidious. Most of the patients have the history of CHD and when the chronic nephropathy in most of such patients is detected,it is advanced to stage 5.The renal injury induced by Guanxinsuhe pill usually presents as chronic tubulointerstitial nephropathy, renal tubular dysfunction and anemia.Anemia is of more severity than renal dysfunction. The 24 hours urinary protein excretions of all the patients are lower than 1 000 mg daily. It is suggested to avoid taking Guanxinsuhe pill, adjust the prescription of Guanxinsuhe pill and do not use slander dutchanspipe root as an ingredient.

ABSTRACT

Objective The aim of this study was to determine whether psychomotor performance and visual reaction time were affected by acute exposure to mild or moderate hypoxia. Method Eighteen healthy male volunteers performed finger tapping, simple reaction time(SRT) and 4-choice reaction time(CRT) tests at simulated altitude of 300 m (control),2800 m, 3600 m and 4400 m for 1 h in a hypobaric chamber. Result SaO2 decreased from 98%(control) to 90%,82% and 74% respectively at the various altitudes. All the performance parameters showed no significant change after exposure to 2800 m for 1 h relative to ground level(P>0.05). However the mean reaction time of 4-CRT under 3600 m prolonged and performance decreased as compared with baseline value(P<0.05), and the performance decreased further under 4400 m(P<0.01). No significant difference was found in finger tapping and SRT even under exposure to 4400 m for 1 h.Furthermore, no decrease in correct rate were observed at any altitude (P>0.05). Conclusion The results from this study demonstrated that there were no measurable impairment of visual reaction time and psychomotor performance under exposure to an altitude of 2800 m for 1 h. However, adverse effects on psychomotor performance were observed under 3600 m and over.
